Deliver nursing in people's homes in Darwin, supporting older clients and NDIS participants. The permanent full-time community care position calls for clinical judgement alongside an approach tailored to each person's health, goals and independence. You will assess needs, review care plans and communicate with clients, carers, families and other professionals.
Candidates need to be willing to move to the Northern Territory. Visa sponsorship is available for the right candidate.
Use an English CV to show community nursing experience and AHPRA registration. prepare a home and community nursing CV with examples of medication management, clinical assessment and safe person-centred practice.
Key Responsibilities
- Provide clinical care for aged care clients and NDIS participants and conduct assessments to develop and review care plans.
- Help manage medication and watch for changes in clients' health.
- Keep clinical records accurate and collaborate with clients, families, carers and healthcare professionals.
- Encourage client choice, independence and dignity.
Requirements & Eligibility
- Current AHPRA registration as a Registered Nurse, with relevant community nursing, aged care or disability services experience.
- Strong skills in clinical assessment, communication and organisation.
- Confident working independently in the community, with a compassionate approach committed to safe, person-centred care.
- Willing to move to the Northern Territory.
