Compassionate, experienced Care Partner / Case Managers are wanted by an aged care provider whose clients are older Australians living at home under the Support at Home program. The close-knit team supports clients across Sydney.
You face clients directly, leading care coordination and managing each person's support plan. The team has a flat structure, so you work on your own initiative while checking that clients get the right services when they need them. Visa sponsorship is also available for the right candidates.
Case management is judged on judgement, so show how you have coordinated services, run assessments and led support workers. Add your knowledge of My Aged Care and keep the English plain, then build it with Build AI CV's guided layout.
Key Responsibilities
- Develop respectful, strong working relationships with clients, their families, support workers and outside providers
- Arrange support services tailored to each client's goals and health conditions
- Assess clients, then draft and revisit their care plans
- Choose courses of action that put safety, wellbeing and quality of life first
- Guide and supervise Support Workers so service stays consistent and of good quality
- Track and manage client budgets while complying with legislation, funding guidelines and internal policies
- Write accurate, timely progress notes and go over the notes of support workers, suppliers and allied health professionals
- Bring on new clients through intake assessments and meet and greets
Requirements & Eligibility
- Proven background in aged care, home care, community services or a health setting
- Skilled in assessment, care planning and case management
- Self-assured working independently and juggling competing priorities
- Excellent at communication, building relationships and leading others
- Working knowledge of My Aged Care and the Aged Care Quality Standards
- Driver's licence, own vehicle and willingness to travel across Sydney
- A strong advantage: bilingual or multilingual ability. A beneficial addition, not required: a clinical background such as nursing or allied health
